The ideal male body shape is considered to be trim and athletic-looking, with broad shoulders and a broad chest, a flat abdomen, and a narrow hip-thigh ratio. However, as men age, areas of fat tend to accumulate around the abdomen, the flanks ("love handles"), along the chin and neck, and finally the breast area or gynecomastia.

Male breast enlargement or gynecomastia is defined as the development of enlarged male breasts.  Most commonly, between the ages of 12 to 17, the male breast undergoes accelerated development during puberty, but the development is transient and usually subsides over time.  The unwanted effect is protrusion of the areola and/or the breast, causing an undesirable feminine shape. 

Puberal gynecomastia is usually self-limiting and is managed with reassurance and conservative, nonsurgical therapy.  The cause of puberal gynecomastia is not clear although it may relate to a hormone imbalance or unusual sensitivity of the breast tissue to normal levels of circulating hormones.  Appropriate measurement of circulating hormone levels, including estrogen, testosterone, and prolactin should be obtained by your private physician to complete the differential diagnosis.  Surgical management usually is waived until puberty is completed.

However, in older males, gynecomastia may be induced or acquired through the use of exogenous steroids and hormones or by the ingestion of specific types of medications that may interfere with the normal balance of circulating testosterone and estrogen.  Another type of gynecomastia, termed pseudogynecomastia, refers to deposit of fat, instead of breast tissue, in the breast and is commonly seen in obese men.

In many cases, liposuction and liposculpting of the chest wall overlying the pectoralis major muscle can effectively correct gynecomastia through male breast reduction.  However, occasionally our clients may require direct excision of the fibrous portion of the enlarged breast tissue and/or excision of excess skin to provide the best aesthetic result.
